Proof of Address

Document acceptable as Proof of Address

Form 49A & CSF (Residing within India):-

1. AADHAR Card issued by the Unique Identification Authority of India
2. Allotment letter of accommodation issued by the Central Government or State Government of not more than three years old
3. Bank account statement**
4. Certificate of Address in original signed by a Gazetted Officer
5. Certificate of Address in original signed by Member of Legislative Assembly
6. Certificate of Address in original signed by Member of Parliament
7. Certificate of Address in original signed by Municipal Councilor
8. Consumer gas connection card or book or piped gas bill**
9. Credit Card Statement**
10. Depository Account Statement**
11. Domicile certificate issued by the Government
12. Driving License
13. Elector's photo identity card
14. Electricity Bill**
15. Employer certificate in original
16. Landline telephone or broadband connection bill**
17. Latest property tax assessment order
18. Passport
19. Passport of the spouse
20. Post office pass book having address of the applicant
21. Property registration document
22. Water Bill**

Form 49A & CSF (Residing outside India):-

1. Bank account statement in the country of residence**
2. NRE bank account statement in India**

Form 49AA:-

1. Bank account statement in the country of residence**
2. Certificate of residence in India or Residential permit issued by the State Police Authority
3. NRE bank account statement in India**
4. Overseas Citizen of India card issued by Government of India
5. Passport
6. Person of Indian Origin card issued by Government of India
7. Registration certificate issued by the Foreigner's Registration Office showing Indian address
8. Visa granted and copy of appointment letter or contact from Indian Company and Certificate (in original) of Indian Address issued by the employer

Note:-

1. **Should not be more than three months old on the date of the application.
2. Documents must be self attested by the applicant.
3. Documents should be in the full name of the applicant.
4. In case of a person being a minor, any of the above mentioned documents of the parents or guardian of such minor shall be deemed to be proof of address.
